What you need to send us:

We are requesting that all referrals for our residential programs include the following:


  • Signed Release of Information
  • Biopsychosocial assessment/Comprehensive assessment
  • Complete Blood count
  • Insurance information/ Public Assistance or SSD/SSI information
  • Medical history
  • Medication list
  • Microscopic urinalysis
  • Physical exam
  • PPD (Tuberculosis test)
  • Psychiatric history (if applicable)

For Children entering the Women and Children’s program, please include the following documents:

Note: The Women and Children’s Program is temporarily closed. We hope to reopen this program in early 2025. 

  • Birth Certificate
  • Immunization Records
  • Physical
  • Social Security Card
